I need to file an application for graduation. 

Are you planning to graduate in December?

 No, I am going to graduate in May. 

Okay, I will file this with the registrar. If you want a different name on you diploma than what’s on your records, you will have to fill out a green name change form.

 When is the deadline for turning in the green form? 

Be sure to file it 30 days prior to graduation.
